すべてのお客様に、施術前に以下の誓約書に署名いただくようお願いしております。契約書をご確認いただき、予約手続きをすることで契約書の内容に同意されたものとみなされます。

ご登録いただいた住所には会員カードが送付または当日ご予約の時に持参していきます。（発行料500円がかかります。）
カードの到着までには2〜3週間かかる場合がございます。会員登録が完了いたしました時点でご予約いただけます。
